- Asthma Treatment Plan (Required for all students with a diagnosis of asthma)
- Epi-Pen Packet

- Non-Prescription/Prescription Medication Consent
- Emergency Medication Self-Administration Form
- Athletic Physical Exam Form
- Universal Health Record - Physical Exam
Please note the following health-related information:
- Although parents are responsible for obtaining a physical examination for their child, it is the school district’s responsibility to inform parents/guardians of the importance of obtaining physical examinations at least once during each of the student’s developmental stages: early childhood (pre-school through grade 3), pre-adolescence (grades 4 through 6) and adolescence (grades 7 through 12).
- All students will undergo screenings. Screenings are grade dependent and include: height, weight, blood pressure, vision, hearing, and scoliosis. They are completed throughout the year and are not on any set schedule. If you do not want your child screened by the school nurse you must notify the school nurse, in writing or by email, as soon as possible.
- Medical information will be shared on a need-to-know basis with the appropriate faculty/staff.
- All medication must be brought to the nurse’s office by a parent/guardian. Students cannot bring in the medication, unless the student is authorized with written permission by physician and parent to self-carry a life-saving medication as indicated above.

ALL UNUSED MEDICATION NEEDS TO BE PICKED UP BY PARENT/GUARDIAN FROM NURSE’S OFFICE BEFORE LAST DAY OF SCHOOL EACH ACADEMIC YEAR, OR IT WILL BE DISPOSED OF IN ACCORDANCE WITH FEDERAL, STATE, AND LOCAL LAW.
